Responsibilities

What You'll Do:

 

The Clinical Intake Coordinator (CM III) facilitates intake psychosocial assessments, stabilization planning, discharge planning, obtaining collateral, clinical interventions, and crisis management.  This position works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for individuals, their family members, clinics, local providers, and/or other referral sources.  This position will carry a caseload and act as an ambassador to community partners.

 

  • Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner.
  • Actively coordinate care in collaboration with interdisciplinary team members and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
  • Maintains the Electronic Health Record (EHR), including documentation of activities performed as part of the service delivery process. Document all services and individual activities completed in the medical record regarding brief interventions, coordination of care, discharge planning, stabilization planning, and any other pertinent information.
  • Maintains a working knowledge of the Washington State behavioral health system and potential funding sources.
  • Participates in individual and group supervision as required. 
  • Participates in ongoing education, including in-services, training, and other activities to maintain and improve competency.  Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
  • Performs all other duties as assigned.

Qualifications

What You'll Bring:

 

  • Master's degree in behavioral health, Social Work, Counseling, Psychology, or related discipline
  • Unrestricted license as a licensed Mental Health Professional (LMHP) in Washington State and/or Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or associate license (LICSWA, LMHCA, etc).
  • 3 years of experience in mental health, substance abuse treatment, or relevant work
  • Knowledge of the Washington State behavioral health system of care and resources within it
  • Must be able to perform Safe Clinch patient restraint techniques
  • The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment.
  • Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role.

It would be great if you had:

 

 

  • Experience working with children, youth, families, and adults with behavioral health issues
  • Knowledge of and experience with the Washington State involuntary treatment process

The typical hiring range for this role in Washington is $40.50 - $47.75 per hour.
