What You'll Do:

The Case Manager III facilitates admission intakes, treatment planning, discharge planning, obtaining collateral, individual therapy, group counseling, life lesson skills, clinical interventions, crisis management, family therapy, support system development, assists in the legal process and initiates personalized care for individuals who are in crisis and stabilizing from crisis. Works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for individuals, their family members, clinics, local providers, and/or other referral sources. This position will carry a caseload and act as ambassador to community partners.

• Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner.
• Review and sign off on Psychosocial Assessments completed by Certified AAC staff members.
• Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
• Participates regularly in Interdisciplinary Treatment Team meetings.
• Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.
• Gather collateral from the individual and involved parties to provide a thorough assessment of what an individuals' needs are for treatment and discharge.
• Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process. Monitor the petition process to ensure timeliness are adhered to, and forms are complete and thorough. When subpoenaed, attend E&T hearings.
• Actively coordinate care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
• Identify and address gaps in service needs for participating parties in service delivery to the individual and make appropriate recommendations to meet the individuals’ needs upon discharge.
• Collaborate with Interdisciplinary Treatment Team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.
• Conducts brief intervention, individual, family, and group counselling within scope of practice. Participates in redirecting individual behavior while maintaining a safe milieu when necessary. Documents all services and individual activities appropriately.
• Provides internal education regarding DBT, trauma informed care, harm reduction, and service plan documentation to enhance staffs' ability to effectively address and manage crisis situations.
• Provides case management services to promote recovery and community reintegration. Conducts crisis interventions and follow-up as necessary.
• Maintain the Electronic Health Record (EHR), including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals’ activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.
• Prepares clinical reports and individualized service plans utilizing agency electronic clinical documentation software. Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.
• Maintains a working knowledge of the Washington State behavioral health system and potential funding sources.
• Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
• Performs all other duties as assigned.

What You'll Bring:

• Master’s degree in behavioral health, Social Work, Counseling, Psychology, or related discipline 
• Unrestricted license as a Licensed Mental Health Professional (LMHP) in Washington State and/or Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or associate license (LICSWA, LMHCA, etc.)
• 1 years of experience in mental health, substance abuse treatment, or relevant work
• Knowledge of the Washington State behavioral health system of care and resources within it
• The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
• Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role

It would be great if you had:

• Experience working with children, youth, families, and adults with behavioral health issues
• Knowledge of and experience with the Washington State involuntary treatment process

The typical hiring range for this role in Washington is $40.50 - $47.50 per hour.
